Sleep apnea is common, substantially underdiagnosed, and associated with cardiovascular and cognitive outcomes that matter over decades. Home testing is a legitimate clinical tool — with limits that consumer wearables do not acknowledge.
Obstructive sleep apnea causes repeated collapse of the upper airway during sleep, producing intermittent oxygen desaturation and repeated arousals. The immediate consequence is fragmented, non-restorative sleep. The longer-term associations are the reason it appears in a healthspan discussion at all: untreated moderate-to-severe apnea has been associated with hypertension, atrial fibrillation, stroke, type 2 diabetes and cognitive impairment in a large observational literature.
It is also strikingly underdiagnosed. Snoring is socially normalised, daytime sleepiness gets attributed to a busy life, and the people best placed to notice breathing pauses are asleep themselves. A laboratory polysomnogram measures brain activity, eye movement, muscle tone, airflow, respiratory effort, oxygen saturation, heart rhythm and body position, with a technician present. It is the diagnostic reference standard and it can distinguish obstructive from central apnea and detect other sleep disorders.
A home sleep apnea test measures a subset: typically airflow, respiratory effort, oxygen saturation and pulse rate. It is a validated clinical tool for diagnosing moderate-to-severe obstructive sleep apnea in people with a high pre-test probability, and it is far cheaper and more accessible. What it cannot do is reliably rule apnea out.
A lab study calculates the apnea-hypopnea index using measured sleep time, because EEG tells it when you were actually asleep. A home test has no EEG, so it divides events by total recording time and reports a respiratory event index instead.
Since recording time includes the periods you were lying awake, the denominator is larger and the index comes out lower. A home test result therefore tends to underestimate severity, which is exactly why a negative home test in someone with strong symptoms is a reason for a lab study rather than reassurance.

A positive home test is the start of a clinical pathway, not the end of one. Treatment decisions depend on severity, symptoms and comorbidities, and the options — positive airway pressure, mandibular advancement devices, positional therapy, weight management, surgery in selected cases — are all prescribed and fitted rather than bought off a shelf. Anyone selling you a treatment on the basis of a home test result without clinical involvement is skipping steps that matter.
It is also worth knowing that adherence to positive airway pressure therapy is the main determinant of whether it helps, and that early support with mask fit and pressure settings is what drives adherence. That support comes from a sleep clinic, not from a device purchase.
