Hearing loss is the single largest potentially modifiable risk factor for dementia identified by the Lancet Commission. The intervention evidence is more nuanced than that headline — and OTC devices have made the intervention affordable for the first time.
The observational evidence linking hearing loss to cognitive decline is substantial and consistent. The Lancet Commission on dementia prevention, intervention and care identifies hearing loss as the largest single potentially modifiable risk factor in midlife, on the basis of a large body of cohort research.

The honest summary: hearing treatment is not established as a general dementia prevention measure for everyone, and there is meaningful trial evidence that it slows cognitive decline in people already at elevated risk. Given that treating hearing loss also demonstrably improves communication, social participation and quality of life, the case for treating it does not rest on the cognitive question at all.
Over-the-counter hearing aids became available in the United States in 2022 for adults with perceived mild to moderate hearing loss.
