Sample Type Determines What You Are Measuring

Consumer hormone tests use three collection methods, and they are not interchangeable. Understanding which fraction each one measures is the difference between a result a clinician can use and one they will politely set aside.

Blood, whether venous or dried blood spot, is the reference method for most clinical hormone assessment. Serum testosterone, oestradiol, thyroid hormones, SHBG and DHEA-S all have established clinical reference ranges derived from blood. Dried blood spot collection correlates acceptably with venous draw for many analytes and is what makes at-home blood hormone testing possible.

Saliva measures the unbound, biologically free fraction of steroid hormones, which is genuinely interesting and less clinically established. Its clearest use is multi-point cortisol sampling across a day, because collecting four saliva samples is trivial and collecting four blood draws is not. Salivary sex hormone testing has weaker clinical standing.

Dried urine panels measure hormones and their metabolites over a collection window, which shows how the body is processing hormones rather than just how much is circulating. That metabolite information is the genuine selling point. It is also the sample type furthest from standard clinical reference ranges, so results frequently need interpretation by a practitioner familiar with the specific panel.

Timing Is Half the Result

The Rules That Make a Hormone Result Interpretable

Testosterone and cortisol: collect in the morning, ideally within an hour or two of waking, fasted. Both follow a pronounced diurnal rhythm, and reference ranges assume a morning sample.

Female sex hormones: the cycle day determines the expected range entirely. Progesterone is typically assessed around day 21 of a 28-day cycle; testing on the wrong day produces a normal result that looks abnormal or the reverse.

Thyroid: pause high-dose biotin for several days beforehand. Biotin is a well-documented interferent in many immunoassays and can push thyroid results in misleading directions.

Everything: avoid testing during acute illness, immediately after intense exercise, or after a night of poor sleep, all of which move hormone levels enough to matter.

What a Result Can and Cannot Settle

A home hormone panel is a screening and tracking instrument, not a diagnosis. Low testosterone, for instance, is diagnosed on the basis of consistent symptoms plus at least two morning measurements on separate days, interpreted alongside clinical context — not from a single kit result. Thyroid dysfunction similarly requires repeat testing and clinical correlation.

That matters because the treatments involved are serious. Testosterone replacement and thyroid hormone are prescription therapies with real risks and real monitoring requirements, and neither is an appropriate response to one number from a mail-in kit. The right use of a home panel is to establish whether a conversation with a clinician is warranted, and to arrive at that conversation with data.
