What DHEA Actually Is — and Why It Matters

DHEA (dehydroepiandrosterone) and its sulfated form DHEA-S are produced primarily by the adrenal zona reticularis — the innermost cortical zone of your adrenal glands, sitting just above your kidneys. Smaller quantities come from the gonads and, importantly, from local synthesis in peripheral tissues including the brain, skin, breast, and prostate.

Circulating DHEA-S functions as a reservoir. Peripheral tissues convert it on-demand into biologically active androgens (testosterone, androstenediol) and estrogens (estrone, estradiol) via local enzymatic machinery. This tissue-specific hormone synthesis — operating independently of the gonads — is called intracrinology, a concept pioneered by endocrinologist Fernand Labrie at Laval University. It means that even in postmenopausal women or men with lower testicular output, peripheral DHEA conversion remains a meaningful source of sex steroid activity at the tissue level.

This distinction matters clinically. Blood testosterone or estrogen levels do not capture what is happening inside tissues. A woman's breast or vaginal tissue may be converting DHEA into active estrogens even when serum estrogen reads as undetectable. This intracrine action explains much of DHEA's tissue-specific effects and underlies why vaginal DHEA (Prasterone) received FDA approval without significantly elevating systemic hormone levels.

DHEA-S as a Biological Age Biomarker

Population studies have repeatedly identified DHEA-S as a correlate of biological aging and longevity. What the Clinical Evidence Actually Shows

The DHEA supplementation literature is large, sometimes contradictory, and requires careful parsing. Here is what the highest-quality evidence supports:

Vaginal Atrophy and Sexual Function — FDA Approval

Fernand Labrie's decades of intracrinology research culminated in the FDA approval of Prasterone (Intrarosa) — vaginal DHEA — for dyspareunia (painful sex) due to menopause-associated vulvovaginal atrophy. The pivotal Phase 3 trials showed significant improvements in vaginal tissue, lubrication, and sexual function with 0.5% DHEA suppositories inserted nightly. Crucially, systemic hormone levels (estradiol, testosterone) remained within normal postmenopausal ranges, demonstrating the intracrine mechanism in action.

Bone Density — The Jankowski and Nair Evidence

Jankowski et al. (2006) randomized older women to 50 mg/day oral DHEA for 12 months and observed significant increases in lumbar spine bone mineral density versus placebo — with no significant change in hip BMD. The landmark Nair et al. (2006) NEJM study from the Mayo Clinic gave 50 mg/day DHEA to elderly men and women for two years. Results: older women showed significant improvements in bone density and muscle strength. Men showed less consistent benefit. Mood and Depression — The Wolkowitz Data

Owen Wolkowitz at UCSF conducted multiple trials of DHEA in patients with major depression, finding significant antidepressant effects particularly in those with low baseline DHEA-S levels. A NIH-funded crossover trial confirmed that DHEA produced robust improvements in depressive and anxiety symptoms in patients with midlife-onset dysthymia. The effect size was clinically meaningful — comparable to standard antidepressants in this population. In contrast, trials in non-depressed or non-deficient populations show far weaker or null effects.

Cognition — Mixed Picture

The Barrett-Connor cohort data, while showing survival benefits in men, did not demonstrate significant cognitive preservation with higher DHEA-S in a general healthy population. RCTs of DHEA supplementation on cognition in cognitively normal older adults have been largely negative. The picture is different in states of frank deficiency (Addison's disease, hypopituitarism) — where DHEA replacement consistently improves well-being, mood, and sometimes cognitive performance. The lesson: DHEA supplementation appears most beneficial when correcting a genuine deficiency, not when added on top of already-adequate levels.

Body Composition — Modest Effects

Multiple trials have shown minor improvements in fat mass and lean mass at doses of 50–100 mg/day, particularly in older adults. The effects are modest and not consistently replicated. DHEA is not a performance-enhancing drug in any meaningful clinical sense, and it is banned by WADA in competitive sport precisely because of its androgen precursor status — not because the anabolic effects are large.

Risks, Side Effects, and Who Should Avoid It

DHEA is sold over-the-counter in the United States as a dietary supplement. This regulatory status does not mean it is without risk. As an androgen precursor, oral DHEA can produce androgenic side effects — acne, oily skin, hair thinning (androgenic alopecia), and increased facial hair in women — particularly at doses above 25–50 mg/day or in individuals who are efficient converters.

Estrogen conversion is a consideration for anyone with a history of hormone-sensitive cancers (breast, endometrial, prostate). While intravaginal DHEA largely bypasses systemic estrogen elevation, oral DHEA does raise circulating estrogens in women — a factor requiring individualized risk assessment.

Do not self-prescribe DHEA if you have: a history of hormone-sensitive cancer (breast, endometrial, ovarian, prostate); polycystic ovary syndrome (PCOS — already androgen-excess); liver disease; or if you are pregnant or breastfeeding.
