Microneedling has genuine clinical evidence at professional depths. Home devices operate at a fraction of that depth, which changes both what they can do and how safe they are — in both directions.
Microneedling creates controlled micro-injuries in the skin, triggering a wound-healing response that includes collagen and elastin production — the reason the technique is also called collagen induction therapy. The mechanism is well characterised and the clinical evidence is real, but it is concentrated in specific applications.
The strongest evidence is for atrophic acne scarring, where multiple studies and reviews report meaningful improvement, typically over a series of professional treatments. There is also reasonable support for general skin texture, fine lines and, in some studies, for enhancing the penetration of topical treatments. Evidence for stretch marks and for deeper wrinkles is weaker and less consistent.
Critically, essentially all of that evidence comes from professional treatment at depths of roughly 0.5 mm and above, performed with sterile technique, often with anaesthetic. Home devices operating at 0.25 to 0.5 mm are performing a different intervention, and it is not accurate to transfer the clinical results across.

At 0.25 mm and below, a home device is doing something closer to enhancing topical absorption and providing mild superficial stimulation than to the collagen induction studied clinically. That is not nothing, and it is not what the marketing implies.
Going deeper at home is where problems arise: post-inflammatory hyperpigmentation, infection, tram-track scarring and, in people prone to keloids, considerably worse. Depth is not a dial to be turned up for better results.
Dermarollers drag needles across the skin at an angle as the roller turns, which creates a slightly tearing entry and exit path rather than a clean vertical channel. Motorised pens stamp needles vertically in and out, which produces cleaner punctures, more consistent depth, and better control around contoured areas such as the nose and around the eyes.
Pens are the better technology on both effectiveness and safety grounds, and they are also easier to keep hygienic because cartridges are replaced rather than cleaned. Rollers persist mainly because they are cheap, and the cheapest rollers are the ones most likely to have poorly finished needles that bend and cause exactly the tearing damage the technique is meant to avoid.
The most common adverse outcomes from home microneedling are infection and pigmentation problems, and both are largely procedural. Needles must be sterile and single-use or properly disinfected per the manufacturer's instructions; cartridges and roller heads are consumables, not permanent equipment. Never share a device. Clean the skin thoroughly beforehand.
Aftercare matters as much as the treatment. Skin is temporarily more permeable and more sun-sensitive afterwards, so apply only simple, clean products — not acids, retinoids, vitamin C or fragranced serums — for the following days, and use sun protection rigorously. Applying an active ingredient into freshly channelled skin is how irritation and pigmentation problems start.
